Do I have to have the exact date for Revocation of Lost Power of Attorney?
When revoking a 'Power of Attorney,' it asks for the date, which I do not have. I misplaced my copy. Can I just put the year or put June 1, 2006 - August 30, 2006. Do I have to have the exact date it was signed by me? My ex-husband and I signed a 'PARENTAL CONSENT FOR RELEASE OF MEDICAL/EDUCATIONAL INFORMATION AND LIMITED POWER OF ATTORNEY FOR EDUCATIONAL DECISION MAKING'. Can I revoke this without his signature? It says in it 'I understand that this authorization, except for action already taken, is subject to revocation by me at any time. By revoking this does it put just him and I making decisions again.
How do we care for an aunt who has dementia and will not sign a Power of Attorney?
My 83 year old aunt has dementia and will not let anyone in her house but my sister. She refuses to take her medication and will not let anyone have power of attorney. We (family) are worried something will happen to her requiring hospital or nursing care. She has enough funds to pay for assisted living. My 2 sisters and I are her only relatives. How do we handle something like this? She is not a friendly dementia..a nasty one unfortunately with delutions and paranoia. She can still dress herself but the house is a mess. She cannot shop, drive, or do any activities without my sister. I am a medical professional and would like to get health care proxy for her.
What state's Power of Attorney should I use for financial purposes?
I live in New York state and want to grant durable Power of Attorney to an individual in North Carolina, allowing him to conduct certain financial transactions on my behalf relating to specified investment accounts. The investment bank in question is also headquartered in North Carolina but operates nationally. All transactions would be conducted online. Should I use your North Carolina Power of Attorney form, the New York form, or some hybrid version?
