Drivers License

Can I sign my daughter's DMV permit with joint custody?
I have physical custody and joint legal custody of my fifteen year old daughter. She wants to get a driving/learning permit from DMV, but DMV requires both parents signature if there is any joint custody. The problem is her father (my ex) does not sign the permit because my daughter refused to meet/visit him after their argument about a year ago. Even though I made my daughter apologize to him, he still refuses to sign the paper. My question is: if only I sign the DMV's form to get the permit and provide insurance for my daughter, would that be legal enough? Could her father still cause problems regarding this matter? If this is not legal, what is the course of action that I should take?

Does Georgia use offence date or conviction date for driving permit reinstatement?
Does the state of Georgia use the offence date or the conviction date when determining when a persons driving permit can be reinstated? My last DUI was in February of '04, and I did time. My court hearing wasn't until 2 years later (05-20-06). I spent 2 years in prison and they won't grant me a permit to drive.
