Name Change

I changed my last name in Maryland and lost certified copy of the court order. Now I need it in Illinois. What to do?

I was born in Brooklyn, New York. My name on my birth certificate indicates my name as -----. My name should be ------. This is the name I used through my schooling years. I am unable to get a state id or proper social security. I was issued a ss card with --------. Therefore, my name on my birth certificate and my social are different. What type of name change do I need to file? Supreme court or civil court name change?

I had my name changed legally in the mid 90's. I lost all my important documents. I now need proof of my name change for a visa related issue. I was born in NYC but had the name changed in New Jersey while I was living there. How do I obtain a copy of my name change?

My ex-wife remarried and moved to Louisiana with my son. I live in Washington state. I have found evidence of a last name change for my son with out my consent. Can she do this? It is my son's identity and who he was when he was born. I never divorced my son.

My daughter - 15 years old wants to change her last name to her step-father's name. I am opposed until she turns 18 and then she can do what she wants. I have always paid the child support as regulated tried to maintain all visitation rights, and my-Ex wants me to continue to pay child support as directed in the papers. My daughter moved state to state with my-ex for about ten years average of 2000 miles away. However, I maintained continual contact to include her grandparents, gifts, presents, phone calls, etc. They now live in Oregon and filed a petition for her to change her name to the step-father's but do not want to adopt. She has two other younger sisters which have the name of what she wants to change to. What is the normal ruling on this as we will send in objection papers. I prefer her not to do it at all - but going to court we are. Thank you.
