Name Change

I was divorced several years ago,after paying my lawyer an enormous amount of money, andI never even got my previous last name thru the courts. Can I file on my own?

In 1887 I lived in California and was devorced, On my legal records I used the name that I gave myself but not born with that name. Now after more than 22 years I have moved back to New York. Even though all my government records..like Social Security,tax forms, Medical records all have the name I used all these years...New York will not give me a drivers license because my birth certificate has my birth name not the name I have used as described above. How can I get my name changed on my birth certificate? Or how can I get the DMV in New York (Suffork County) to give me a license with the name I use for more than 22 years? I hope you can understand what I am trying to ask here. I have many forms with the name I call myself all these years but that did not seem enough for the DMV. Help would be greatly appreciated. Thank you for any assistance you may give me.

I put the wrong father on the birth certificates of both of my daughters. they are now 13 and 15 and would like to legally have their biological fathers name, whom I've been married to for 6 years. We did conduct an independent DNA test for both children. My ex husband who is not the biological father but is on the birth certificate has not been involved for the past 12 years and will not object. I am unsure of his whereabouts but he was served with a copy of the DNA test results 12 years ago and is aware of the fact that he is not the father. Where do i start to not only change the girls' names but also the birth certificates?

i been married with my husband 6 month ago now his name has been change, i want to carry is new name, i filed a petition in the same court i been married for me to carry me husband name they denied it. they don't want to change it in the marriage certificate even thou they have the prove that the name granted to change, is like i should wait to married with the new name , the also they never done that before once you married is sealed. in that case i would like to know should i annul it , get remarried with the new name. or change all my doc in the new name, but the all name will remain the same in the certificate. i want it to change in the marriage certificate, in that case do i'm eligible for annulment.

I am 34 years of age and my sister is 33 years of age. Our step Dad adopted us today ( 10-02-2009 ). the assistant DA signed off on the adoption of both my sister and myself along with the name change for my sister, however, he would not sign off on my name change and filed his papers to the judge on why he would not sign off on m name change. He said it had to do with something on my criminal background check. The only thing on there would be from a DWI charge from 7-8 years ago. He also said that I would need to file a petition to the judge for a hearing and request that the judge sign off allowing me to change my name. I need to know what forms I need to purchase and file.
