Name Change

Can a judge refuse to grant name change due to prior criminal charge?
I am 34 years of age and my sister is 33 years of age. Our step Dad adopted us today ( 10-02-2009 ). the assistant DA signed off on the adoption of both my sister and myself along with the name change for my sister, however, he would not sign off on my name change and filed his papers to the judge on why he would not sign off on m name change. He said it had to do with something on my criminal background check. The only thing on there would be from a DWI charge from 7-8 years ago. He also said that I would need to file a petition to the judge for a hearing and request that the judge sign off allowing me to change my name. I need to know what forms I need to purchase and file. Thank You & God Bless

How do I change my name on my birth certificate to match the name on my social security card?
I was born in Brooklyn, New York. My name on my birth certificate indicates my name as -----. My name should be ------. This is the name I used through my schooling years. I am unable to get a state id or proper social security. I was issued a ss card with --------. Therefore, my name on my birth certificate and my social are different. What type of name change do I need to file? Supreme court or civil court name change?
What involved if there's a typo on my birth certificate and I want to change my last name?
Hello,I am a resident of California living in Colorado due to my husband's military assignment. I want to know what to do with a typo I found on my birth certificate. It has an error on my first name. I requested a certified copy and it does not match everything I have used throughout my life. My name is Melissa - my drivers license, SSN, school and public records and everything else has that on it. However my birth certificate shows Malissa on the certified copy I received. My parents never mentioned this to me and I have always gone by Melissa. This is the only name I have ever known. How can I clear this up? I think there was a typo on my mom's first name on my birth certificate as well. Also, as a side question, can I change my legal last name to my married name at the same time? I have changed my last name through the SSN but would I need to change it with an official name change too?
