I have the State of Arizona that will be devoloping a highway on rental property in which I live on and also have more people involved with this right of way. I'm trying to find a legal form that would cover the owners and as well as the renters of this property that would be a petition which we could all sign that would be sent to MCDOT there is about 75 to 100 homes affected by the right of way . Changes may take affect as soon as 2012 or sooner. Can you advise me of a form that would cover all the area and also be able to be a petition, or something comparable?

Do I have to show up in person to fill out a pistol permit, or can I download a form online, fill it out, and turn it in to the courthouse? Also, what information will I need to bring with me when I turn in the application (driver's license, car tag number, etc.)?

About 2 years ago, my husband and I allowed my mother to place her mobile home on our property. She pays no rent and there is no contract. She has since moved in with a male friend and only comes by to feed her animals. She has elected to sequester herself from the family and she wants to sell her mobile home; which could take a while. I am not willing to let it stay for a long period of time. Due to certain uncomfortable situations, we want her to move her mobile home from our property since it is not her primary residence any longer. We will, of course, allow a certain time frame to do so, but not an endless amount of time. I would like a form to do this but was unable to find one because it is not lack of rent or maintenance.

Is there a consent form that an emplyee must sign prior to random drug testing? Or can an employer soley pick on one person and make them take a drug test out of the blue without a signed consent form from the employee?
