How do I give up my parental rights? What forms do I need to fill out?

I have been married 5 yrs. Two and one-half years ago my husband and I were going to separate and I had a brief affair. I found out that I was pregnant and told both my husband and the other man. The other man hung up on me and refused to speak to me even after I sent him two certified letters. My child was born and the other man has had nothing to do with my child for over a year and a half. I was wondering if he could still try to get rights or not. My husband is listed on the birth certificate.

Jane and Tom were only dating when Jane became pregnant with Tom's child. Since the birth of the child, Jane and Tom are no longer dating. Jane is not able to raise the child on her own without the financial support of Tom. Jane has been contemplating whether she has the legal right to seek support from Tom, though they have never been married. Can Jane get support for her child even though she has never been married to the father of her child?

I'm interested in signing over my parental rights. What exactly do I need to do? I have been supporting my children (their mother has custody) since they were born. I'm required to have separate medical coverage when it's not even used. I can't enroll in school due to back support I owe.
