I started school in 1954 without a birth certificate, under the name: __________. My name on my birth certificate is ______________. All my records are under ________________ Except my birth certificate. what must I do too have my birth certificate change, to ___________

Will the state allow a woman to change back to her maiden name, even though she stays married ?

Can I change my name to my married name on my license, passport, with creditors and such if I do not change my social security card? I want to use my maiden name for work and the only way I can do that per my job is if my social security card does not change. I want to be known for all other nonwork purposes by my husbands name. The marriage license has my maiden name and husbands name listed as last name with no hyphen.

My child is 5 months old and his biological father is on the birth certificate and the child has his fathers last name. I would like to change his last name to mine considering the fact that the father suffers from the depression and anxiety is never there for the child and would not even let me get the passport for the baby. I have two questions: can I legally change the whole name of the child without the consent of parent considering the fact that he is not a normal person and that we are not together? Is there any way to take him off the birth certificate and how?

What are the laws on a minor name change, I was never married to his father and I gave him his fathers last name, now I am expecting another child and about to be married and I would like my son to have the same last name as us, he doesn't see his dad at all, his dad hasn't looked for him in two years, the only father my son knows is my fiance, my son is now 2 1/2 yrs old.
