Name Change

Can we put baby's father on the birth certificate without giving the baby his last name?
My daughter (16 yrs old) is expecting a baby in December. My daughter and the baby's father want to continue to date and he wants to be a part of the baby's life. My question is.. Can we put him on the birth Certificate as the father without giving the baby his last name? And also if they do indeed get married some day, how difficult would it be to change the baby's last name?
What would the reason for my name change be if I just wanted a new name?

I have an evidence hearing set . Do I need an attorney?
I divorced in California before my son was a year old. In the divorce I was awarded full legal and physical custody of my now 11 year old son. I remarried when my son was almost three and he has always gone by my husbands last name. He sees his biological father once maybe twice a year, he does pay child support. Per the request of my son, I finally filed for a legal name change so his last name legally matches mine and my husbands. My ex-husband came to the hearing and objected to the name change even though he has known my son has gone by this name for nine years. We now have a Evidence Hearing set. Do I need an attorney? What evidence do I need to prove this is in the best interest of my son? and finally Why must I prove this is in his best interest if I have full Legal and Physical custody and my son is there to state his wants?

My husband and I had our first baby before we were legally married. At the baby's birth my husband thought that my maiden name had to go on the birth certificate. Basically my husband and I want to change our son's last name from my maiden name to our married name. How do we go about changing the baby’s name?
When am I required to change my driver's license if my name has changed?
I just legally changed my last name from Smith back to Jones. Double name was causing too much confusion. Smith is the last name of my 1st husband and my 6 children. I'm still married to Jones. Question is: Am I Mrs. or Ms?? Probate court couldn't give me an answer. They said I'm married so I could use Mrs., but I'm technically no longer Mrs. Smith. Also should I change my driver's license right away or wait until it comes up for renewal?
