Is a Parent Required to Consent to Name Change of Minor in Louisiana?
My ex-wife remarried and moved to Louisiana with my son. I live in Washington state. I have found evidence of a last name change for my son with out my consent. Can she do this? It is my son's identity and who he was when he was born. I never divorced my son.
How Do I Change My Name in Georgia?
I got married in 2006 and changed my surname to my husband's surname and my middle name to my maiden surname. This is the naming convention in the Philippines. At that time, I was still a US permanent resident. I had my social security, credit cards, bank accounts, insurance, driver's license and US green card (Permanent Resident Card) changed to assume this name. Last week, I got naturalized as a US citizen and was told that I can only use my birth middle name and no other. Also, I just have to drop my maiden name and take on my husband's surname. I was also asked to sign including my full birth middle name which I have never done before. So this signature never appeared in any of my records. I was advised by the immigration officer that I can have my name changed in the Superior Court later. Now, I want to use my birth middle name and hyphenate my last name so that my maiden and husband's surnames are both there. Does this entail a name change in the court? If I file with the court, what name should I put in the petitioner - the legal name that the USCIS told me or the assumed name? Can I add the 'also known as name' next to the legal name as the name of the petitioner? I want to change all my records to my birth first name, birth middle name and maiden name hyphenate husband's surname but I have two sets of names used now, (1) legal name given by USCIS - birth first name, birth middle name and husband's surname and (2) assumed legal name prior to naturalization - birth first name, maiden name as middle name and husband's surname.
How Effective is an Objection to a Minor's Name Change?
My daughter - 15 years old wants to change her last name to her step-father's name. I am opposed until she turns 18 and then she can do what she wants. I have always paid the child support as regulated tried to maintain all visitation rights, and my-Ex wants me to continue to pay child support as directed in the papers. My daughter moved state to state with my-ex for about ten years average of 2000 miles away. However, I maintained continual contact to include her grandparents, gifts, presents, phone calls, etc. They now live in Oregon and filed a petition for her to change her name to the step-father's but do not want to adopt. She has two other younger sisters which have the name of what she wants to change to. What is the normal ruling on this as we will send in objection papers. I prefer her not to do it at all - but going to court we are. Thank you.
Name Change in Missouri
I want to change my last name to my mother's maiden name. I currently reside in Missouri and in 1995 I filed chapter 13 while living in Texas. The case was paid and complete in 2000, I have proof (documentation) that it was complete. It's been 14 years and this is no longer in any of my credit reports. My question is, do I need to hire an attorney in order to change my last name or can I simply order an inexpensive name change package that are available on-line? I am not working now and I am unable to afford an expensive attorney. Thank you for your time.
How do I have my daughter's last name changed since I was found to be the father?
I have a daughter that is 9 years old. At the time of her birth there was question of paternity because her mother was married to another man. The mother is since divorced from said husband, paternity was settled identifying myself as her father and I now have joint custody of my daughter. The former husband executed a 'Denial of Paternity and Release of Rights' and was excluded as the father of said child through a paternity test. My daughter still carries the former husbands last name. Can I legally change her last name to reflect mine? Where do I start?
