Where can I get an affidavit for collection of personal property?

Full question:

My deceased mother has a 401K and didn't designate a beneficiary, however, I took care of all other matters (daughter). I was instructed by her former employer to send a copy of the estate papers (there are none, as she didn't have a will) or get an affidavit for collection of personal property but didn't find one online. What do I do?

FAQs

If there is no designated beneficiary for an asset like a 401K, the funds typically become part of the deceased's estate. In this case, state intestacy laws determine who inherits the assets. Generally, this means that close relatives, such as children, parents, or siblings, may inherit the funds. However, the exact distribution can vary by state law.
