Where can I find a lease purchase agreement for renting out my home?

We are considering renting our home in PA with the option of purchase. We need something in writing to cover both us as the landlord and the renters that they are responsible for all repairs and upgrades to the property. At which time they pay the asking price of the property, we give them a clear deed OR if they can not pay the asking price and decide to leave the property, they receive nothing in return for any investment they made while renting the property. Do you have such an agreement?
