I have been married for 5 years. I never got my last name changed on my ss card but I did on my drivers license and I have been using my husbands last name on everything except for taxes. I have not worked since we have been married. we got married in Oklahoma and now live in California. I want to get my last name changed on my ss card now and I know that I have to have a recent document with my maiden name on it but I don't have anything except for an old college id card with my name and picture on it and it is not recent. Can I still get my last name changed on my ss card? How do i do this?

Answer:

You may use a copy of your marriage certificate to request a name change on your social security card. A document supporting a name change must be recent and identify you by both your old and new names. If the name change event occurred over two years ago or if the name change document does not have enough information to prove your identity, you must also provide documents to prove your identity in your prior name and/or in some cases your new legal name. You may be able to apply for a state i.d. card or passport if more recent identification is required.
