What can I do about someone who is harrassing me?

Full question:

I have been harrassed since Jan. 07 by a woman that I know. I am dating her soon to be ex husband. She persistently called my home, left obscene messages, came to my door late at night. When I changed my phone numbers, she resorted to calling my workplace. Because she is also employed at my workplace, I reported the harrassment and was transferred to another area. This woman has since resorted to calling vendors, businesses, churches, etc. using my name, setting appointments at my home, always giving my work phone number as a contact point. My supervisors are becoming annoyed at all the personal calls that they must intercept. I dont know if there is a way to prove that she is doing this, so I adamantly document everything. My boyfriend is also recieving the same kind of harrassment, but not to the same extent. While this all may seem harmless, I am fearful for my safety and that of my children.
